AccentCare, Inc. in Sacramento, CA is seeking a Clerical Territory Patient Admissions Coordinator for a full-time, onsite role. You will coordinate with referrals, sales and clients to process quality referrals and document all steps in EMR systems.

Requires at least one year in home care and EMR use; payors experience is a plus. Schedule is Mon-Fri 8am-5pm; salary range $23-$28 per hour. Join a mission-driven team focused on excellent patient outcomes and seamless admissions.

Qualifications

• High School or GED required.
• Minimum of one year experience in home care, hospice, or personal care services.
• Prior use of EMR; HCHB experience preferred.
• Experience handling Medicare, Medicaid and commercial payors preferred.

Responsibilities

• Collaborates with Referral Source, Sales teams, Clients and local operations to collect information for a quality referral.
• Documents referral information and actions in the medical record; completes required workflow tasks within set time frames.
• Ensures referrals from eFax, phone, or portal are addressed promptly.
• Requests and collects documents needed for ICD-10 coding by the CPOCC team when missing.
• Performs follow-up calls to confirm information and resolve any gaps.

What You Need to Know

The Territory Patient Admission Coordinator establishes and maintains communication with the clients, family members and referral sources, in a given territory. This role is responsible for partnering with agency staff, referral sources, and sales to achieve optimal patient satisfaction & outcomes. The Territory Patient Admission Coordinator is responsible for the timely acceptance or decline of the referral based off the predetermined criteria set by the Agency and facilitates obtaining all information and documentation necessary to complete a quality referral. This position’s assigned territory, may encompass one or more states in AccentCare’s operation and may change with the needs of the business.

• High School or GED
• Minimum of one year experience in home care, hospice, or personal care services, required
• Prior use of Electronic Medical Records (EMR), HCHB preferred
• Experience in dealing with a variety of payors in healthcare, e.g., Medicare, Medicaid, and commercial payers, preferred

Responsiblities

• Collaborates and maintains effective communication with the Referral Source, Sales teams, Clients and/or local agency operations to collect needed information to process a quality referral.
• Documents all referral related Information, actions taken, and follow-up required in the medical record in a timely and accurate manner. As applicable utilizes the agency’s software solution to facilitate the referral process by completing required workflow task within the Company’s established time frames.
• Ensures timely attention to referrals received from a variety of platforms, e.g., eFax, telephone, or referral portal/Hospital EMR portal.
• Where applicable, reviews Eligibility Alerts and Responses for Medicare Clients.
• Requests, H&P, Discharge Summary, and other documents needed for ICD 10 coding by the CPOCC team, if not received with the referral information.
• Completes a "Follow Up" call to the Client/Caregiver/Referral Source to collect or confirm information. Example: confirms out of pocket expense, clarify teachable caregiver, and collect missing demographic and physician information.
