Job summary

AccentCare, Inc. is seeking a Territory Clinical Manager in Burnsville, MN. This full-time role oversees patient care services and clinical staff in home health, coordinating an interdisciplinary team to ensure quality and compliance.

The ideal candidate holds an RN license in the operating state, with 3–5 years of RN experience and CPR certification. Travel to multiple locations is required; schedule is Monday–Friday, 8 AM–5 PM.

Qualifications

• Must be a graduate from an approved school of professional nursing
• Licensed to practice as an RN in the state of operation
• 3–5 years of RN experience; home health or related settings preferred
• CPR certification required; valid driver with insured vehicle and able to travel to locations

Responsibilities

• Supervises day-to-day clinical and office operations.
• Provides staff and patient availability during hours of operation.
• Ensures services comply with professional standards, policy and regulatory requirements.
• Coordinates referrals and patient care with an interdisciplinary team.
• Evaluates quality of clinical care and patient needs.
• Collaborates with clinicians and physicians on transfers or discharges.
