AccentCare, Inc. is seeking a Patient Care Manager in Burnsville, MN. This full-time role leads clinical staff to deliver high-quality home health services and coordinates patient care within regulatory standards.

You will supervise nursing and clinical personnel, ensure safety protocols, and drive quality improvement initiatives while fostering a supportive team culture and excellent patient outcomes.

Competenze

• RN license required with current practice in Minnesota.
• Two years of management or supervisory experience preferred in home care.
• Knowledge of professional standards, Medicare Conditions of Participation, and regulations.

Mansioni

• Provide clinical supervision to ensure patient care meets standards and policies.
• Coordinate, plan, and monitor patient care across the team.
• Supervise clinicians to ensure consistent, appropriate services.
• Maintain clear communication with patients, caregivers, referrals, field and office staff.
• Support quality outcomes by meeting goals and participating in quality initiatives.
• Model professionalism and patient-focused service in daily work.
• Uphold safety and compliance through training, reporting, and audits.
• Foster staff development via coaching and continuing education.
