AccentCare, Inc. is seeking an RN Case Manager to oversee patient care plans from admission to discharge in Burnsville and surrounding areas. The role combines direct care and coordination, guiding a care team to ensure quality outcomes.

A $5,000 sign-on bonus is available, with a Mon-Fri 8:00 a.m.–5:00 p.m. schedule and a pay range of $87,000–$97,000 annually. Requirements include MN RN licensure, at least 1 year of RN experience, and CPR certification.

Qualifications

• RN license in the state of operation.
• Minimum one year of RN experience.
• Current CPR certification required.

Responsibilities

• Assess nursing needs and manage the patient's care plan.
• Complete initial patient assessment at admission with history.
• Select appropriate nursing diagnoses.
• Assess medications for ineffective therapy or interactions.
• Set realistic, measurable goals aligned with diagnosis.
• Inform physician of assessment results and interventions.
• Implement nursing interventions per diagnosis and goals.
• Re-evaluate patients' nursing and clinical needs as required.
• Document assessments, progress, discharge notes timely.
• Notify physician and staff of changes in condition.
