AccentCare, Inc. is seeking an RN Case Manager for Home Health serving Burnsville and surrounding areas. The role combines direct and indirect patient care, guiding RNs, LPNs, CHHAs and other clinicians to execute the patient’s plan of care from admission through discharge.

The ideal candidate will have RN licensure in the operating state, CPR certification, and at least one year of RN experience. The position offers a stable weekday schedule and a competitive annual pay range.

Competenze

• Graduate from an approved school of professional nursing.
• Licensed to practice as a registered nurse in the state of operation.
• Minimum of one year of RN experience.

Mansioni

• Assess the status of nursing needs and manage the patient’s care plan.
• Complete initial patient assessment on admission with patient history.
• Identify appropriate nursing diagnoses and review medications for safety.
• Set realistic, measurable goals aligned with diagnosis and inform physician of findings.
• Implement nursing interventions within the Nurse Practice Act and reassess needs as required.
• Document assessments, progress, discharge notes, and required paperwork in a timely fashion.
