Job summary
BAYADA Home Health Care in Caldwell, New Jersey is seeking a Home Health Physical Therapist to perform home visits for adult and geriatric clients. The PT will assess mobility, develop care plans, and collaborate with a multidisciplinary team to help patients remain living at home.
One year of PT experience is required. Base pay starts at $60+ per visit, with full-time opportunities and comprehensive benefits.
Qualifications
- Current New Jersey Physical Therapist license.
- Minimum of one year of recent work experience as a Physical Therapist.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem solving skills.
- Solid computer skills; experience with electronic medical records (EMR) preferred.
Responsibilities
- Make home visits to clients in designated geographic territories (Essex County area).
- Perform diagnostic tests and measurements of mobility, strength, balance and gait.
- Implement wound care and protective interventions.
- Develop and implement individualized care plans including manual therapy and gait training.
- Assess and revise the PT plan of care with the multidisciplinary team.
- Educate clients and families on rehabilitative care and activities for safety and independence.
- Document observations and interventions using a tablet or EMR.
