Job summary
BAYADA Home Health Care is seeking a Home Health Physical Therapist to serve Orange, New Jersey, performing in-home visits for adult and geriatric clients. The role requires an active New Jersey PT license and at least one year of recent home health experience.

Qualifications
- Current New Jersey Physical Therapist license.
- Minimum of one year of home health PT experience.
- Experience performing home health visits for adults/geriatric clients.
Responsibilities
- Make home visits to clients in designated Essex County territories.
- Perform mobility, gait, balance, and strength assessments and document findings.
- Develop and revise individualized care plans with PT interventions.
- Instruct patients and families on exercises and activities to promote recovery and independence.
- Record observations and evaluations using EMR tools.
