Job summary

Qualifications

• Active NJ Physical Therapist license.
• Graduation from an approved PT program per APTA or AMA accreditation.
• Ability to work independently and manage time effectively.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements including mobility, joint range, balance, and safety assessments.
• Develop and implement individualized care plans with manual therapy, gait/balance training, and other interventions.
• Continually assess and revise the PT care plan in collaboration with nursing.
• Educate clients and family members in rehabilitative care and activities to promote health and independence.
