POSITION OVERVIEW

Under supervision of the Program Director, the Tenant Services Coordinator is responsible for providing support to tenants and maintaining monthly reporting requirements that will ensure compliance with the parameters of the program. Reasonable accommodations may be made to enable individuals with disabilities as defined by the ADA to perform the essential functions of the job. The position is Full-time, 35 hours a week generally, 8:30 – 4:30 Monday – Friday, with schedule adjustments as needed per program necessity.

Pays: $50k per year


KEY FUNCTIONS

  1. Tenant Liaison: Acts as liaison between participants, superintendents, and building managers to ensure all repairs are completed to either the tenant's apartment or the building. Reports all issues with respect to the apartment and/or building to the program director, including incidents or occurrences requiring immediate attention.
  2. Tenant Support and Record Keeping: Provide light touch case management, information, referrals, and advocacy to older adults; perform intakes/assessments under the review of a Licensed Social Worker; conduct home visits and provide case management to Promesa Virginia House constituents.
  3. Conduct benefit and entitlement eligibility assessments, and assist clients with public benefits applications and other entitlement programs and opportunities.
  4. Conduct group workshops & support groups as needed.
  5. Collect and enter all participant and service unit data in database(s) thoroughly and timely, in accordance with organizational policies and regulatory mandates.
  6. Perform tabling and other outreach at neighborhood events.
  7. Assess older adult needs and develop care plan strategies.
  8. Provide support and resources, maintain good relationships with external providers, and develop linkages to community resources.
  9. Research and referrals based on the care plan goals.
  10. Assist with benefits such as Access A Ride or Visiting Nurse services, Meals on Wheels.
  11. Respond to crisis situations, developing follow-up report details and completing and submitting incident reports.

REQUIREMENTS

  1. High School Diploma and 4 years minimum relevant demonstrated experience providing case management services to at-risk populations (i.e., Older adults, Mental Health, or Substance use) Required.
  2. Associate degree Preferred.
  3. Ability to use business acumen to exercise good judgment and decision-making.
  4. Ability to deal with sensitive issues with diplomacy and discretion.
  5. Must be able to multitask with strong organizational skills.
  6. Excellent interpersonal skills and able to communicate both verbally and in written form.
  7. Commitment to cultural diversity and sensitivity.
  8. Ability to maintain a professional demeanor under pressure, and operate with a keen sense of urgency and commitment to quality.
  9. Willingness to perform other related duties as assigned.
