Job summary

Fallon Health is seeking an Assessment Nurse Case Manager to perform face-to-face home visits within 30 days of enrollment and conduct regulatory assessments for new enrollees. The role includes Health Risk Assessments (HRAs) and PCA assessments, with in-person and telehealth options and coordination with the care team.

The position requires an RN license in Massachusetts, a nursing degree, and familiarity with MDS processes.

Qualifications

• 1+ years of clinical RN experience with complex medical, behavioral, and social co-morbidities.
• Ability to conduct assessments in-person and telehealth.
• Strong communication and interviewing skills.
• Knowledge or willingness to learn regulatory requirements.
• Reliable home internet required.

Responsibilities

• Conduits home visits for onboarding and regulatory assessments.
• Completes HRAs and PCA assessments.
• Submits MDS assessments.
• Provides education on NaviCare case management program.
• Conducts telephonic assessments when appropriate.
• Collaborates with Care Team.
• Completes LTSS evaluations and collaborates with UM.
