Job summary

Fallon Health is seeking an Assessment Nurse Case Manager to perform in-home onboarding and regulatory assessments, delivering Health Risk Assessments and PCA evaluations for new enrollees. The role includes telehealth options, coordination with the care team, and timely submission of MDS assessments.

Educate members about NaviCare programs, participate in LTSS evaluations, and support quality initiatives such as HEDIS and Medicare/Medicaid.

Qualifications

• Graduate from an accredited school of nursing.
• Massachusetts RN license active and unrestricted.
• Case management certification strongly desired.
• Valid driver's license and reliable transportation.
• Experience with LTSS, OASIS/MDS, and Medicare/Medicaid preferred.

Responsibilities

• Conducts in-home onboarding and regulatory assessments.
• Performs HRAs and PCA assessments.
• Submits MDS assessments in a timely manner.
• Provides education on NaviCare case management program.
• Conducts telephonic assessments when appropriate.
• Collaborates with multidisciplinary care teams.
• Completes LTSS evaluations and coordinates with UM.
