Qualifications

• Driving with a valid license and reliable transportation.
• Massachusetts RN license required.

Responsibilities

• Performs face-to-face home visits for onboarding and regulatory assessments.
• Completes HRAs, PCA assessments, and MDS submissions.
• Provides education on NaviCare case management program and resources.
• Conducts telephonic assessments when appropriate and collaborates with the care team.
• Maintains knowledge of program policies and supports quality initiatives.
