Job summary

Fallon Health is seeking an Assessment Nurse Case Manager in Massachusetts to onboard new enrollees through home visits and regulatory assessments. You will perform HRAs, PCA assessments, and timely MDS submissions, primarily in person with some telehealth options.

Applicants must be a graduate of an accredited nursing program, hold an active MA RN license, and have at least 1 year of clinical RN experience with complex clinical and social needs.

Qualifications

• 1+ year of clinical RN experience with complex medical, behavioral, and social co-morbidities.
• Ability to conduct assessments in-person and via telehealth.
• Excellent communication and interviewing skills.
• Willingness to learn regulatory requirements.

Responsibilities

• Conduct in-home onboarding and regulatory assessments.
• Perform HRAs and PCA assessments.
• Submit MDS assessments on time.
• Provide education on NaviCare case management program.
• Conduct telephonic assessments when appropriate.
• Collaborate with Care Team.
• Complete LTSS evaluations and coordinate with UM.
