Job summary

Fallon Health in Massachusetts seeks an Assessment Nurse Case Manager to perform in-home visits and regulatory assessments for new enrollees. The role includes conducting Health Risk Assessments, managing PCA assessments, and ensuring timely MDS submissions, with some telehealth as needed.

You will collaborate with the care team, educate members about NaviCare, and participate in LTSS evaluations and UM coordination.

Qualifications

• Graduate from an accredited school of nursing; BSN or higher preferred.
• Active, unrestricted Massachusetts RN license.
• Certification in Case Management strongly desired.

Responsibilities

• Conducts home visits for onboarding and regulatory assessments.
• Completes HRAs and PCA assessments.
• Submits MDS assessments.
• Provides education on NaviCare case management program.
• Conducts telephonic assessments when appropriate.
• Collaborates with Care Team.
• Completes LTSS evaluations and collaborates with UM.
