Hildebrand Family Self-Help Center, Inc. seeks a Temporary Part Time Stabilization Case Manager to support families exiting the shelter and maintaining housing stability in the greater Boston area.

Qualifications

• Associate’s degree or higher in Human Services or Social Work preferred.
• Experience working independently but also collaboratively.
• Bilingual Haitian Creole or Spanish.
• CPR/SFA Certification within 30 days of hire.
• Use of a personal vehicle or reliable transportation for home visits.

Responsibilities

• Support families exiting Hildebrand Emergency Shelter for 24–36 months.
• Develop initial stabilization plans and update every 30 days.
• Monthly landlord communications to ensure rent is paid and address issues.
• Document stabilization efforts and progress in ETO/ASIST.
• Track client demographics, income, immigration status, and Hildebrand tiering data.
