Valle Vista Health System in Greenwood, IN is seeking a Discharge Planner to coordinate discharge planning for patients on acute behavioral health units. You will work with patients and families to develop safe post-discharge plans and connect them with aftercare services in the community.

A 2-week mandatory FT orientation is required. Prior experience with psychiatric in-patient populations is preferred and a Bachelor's degree in social work or related field is required.

Position Summary

This employee is responsible for the discharge planning for patients receiving care on the acute mental health units. The Discharge Planner provides patients with a review of treatment options beyond Valle Vista Health System. The Discharge Planner represents VVHS in coordination of treatment to follow up for patients within the community. The Discharge Planner works closely with other members of the social services team from the time of intake to plan an appropriate discharge plan.

Training

A mandatory FT 2-week orientation class is required.
