Job summary

Central Florida Behavioral Hospital, a 174-bed private psychiatric center in Orlando, seeks a Discharge Planner to ensure safe transitions from inpatient care to community-based services. You will coordinate post-discharge services, review plans with patients, and educate families to support continuity of treatment.

Youll work with therapy, housing, medication management, and community supports to reduce readmission risk, focusing on clear communication and coordinated care.

Qualifications

• Bachelor’s degree in social work or related field required.
• Two years’ experience in a psychiatric setting preferred.
• Handle with Care, Verbal Deescalation and BLS/CPR required (may be obtained upon hire).
• Effective oral and written communication skills.

Responsibilities

• Arrange and follow up on post-discharge services such as therapy, medication management, housing, and community support.
• Meet with patients to review the discharge plan and ensure patient understanding.
• Ensure patients leave with prescribed medications and refills, and understand how to access them.
• Involve patients and their families in planning, providing education, and addressing questions in understandable language.

Position Summary

The Discharge Planner is responsible for ensuring a smooth, safe, and effective transition of patients from inpatient behavioral health treatment back into the community. This role bridges the gap between clinical care and aftercare, coordinating services to maintain continuity of treatment and reduce readmission risk.
