Valle Vista Health System in Greenwood, IN is seeking a Discharge Planner for a 12-week temporary assignment. You will lead discharge planning on acute mental health units, reviewing treatment options and coordinating follow-up care with community providers. Qualifications

Position Summary: Days (Temporary 12-week Assignment)

This employee is responsible for the discharge planning for patients receiving care on the acute mental health units. The Discharge Planner provides patients with a review of treatment options beyond Valle Vista Health System. The Discharge Planner represents VVHS in coordination of treatment to follow up for patients within the community. The Discharge Planner works closely with other members of the social services team from the time of intake to plan an appropriate discharge plan.

Work Hours: Monday - Friday 9:30 am to 6:00 pm (November 1st to February 1st)

• Work directly with patients and their families to ensure patients have appropriate safe plans after discharging from acute behavioral health hospital
• Coordinate aftercare services with community providers.

REQUIREMENTS:

Education:

Bachelor's degree in social work or other social science is required.

Experience

Six months to one year of experience working with a psychiatric in-patient population preferred.
