Job summary

Hickory Trail Hospital (a UHS facility) seeks a Discharge Planner to coordinate discharge planning and develop post-hospitalization care plans. You will work to ensure continuity of care while coordinating with patients, families, and care teams.

The role includes acting as Court Liaison with Mental Illness Courts for OPCs, MIWs, APOWWs, and court-ordered medications. A Master’s in Social Work or related field is preferred; CPIs within 90 days are required.

Qualifications

  • Master’s Degree in Social Work or a clinical related mental health field preferred.
  • Minimum of three (3) years’ experience in a psychiatric health care facility with direct experience in discharge planning.
  • Crisis Prevention Training (CPI) within 90 days of employment and prior to assisting with a restraining procedure.

Responsibilities

  • Coordinate discharge planning activities and develop a plan of care to meet post-hospitalization needs.
  • Serve as Court Liaison handling documentation with Mental Illness Courts on OPCs, MIWs, APOWWs, and court-ordered medications.
  • No supervisory responsibilities; ensure continuity of care across transitions.

Hickory Trail Hospital, a 86-bed behavioral health facility located just south of Dallas, has been providing quality health care to the residents of DFW since 1988. Hickory Trail Hospital provides inpatient and day treatment for children, adolescents, adults, and senior adults. Our professional staff individualizes care to uniquely fit the intensity and severity of each client’s case and their family’s needs. Because crisis occurs at all times of the day and night, our services are provided 24 hours a day, 7 days a week.

Our Outpatient program (“Better Days” Day Hospital PHP/IOP) offers treatment for adolescents and adults Monday - Friday. Better Days has earned exceptionally high client satisfaction scores, as we have compassionate, knowledgeable, and dedicated therapists who take pride in their work and focus on assisting clients in successfully reaching their goals.

We concentrate on promoting our mind-body-spirit therapeutic philosophy, ensuring clients are not only connected with our respected psychiatrists to address medication needs for mental health concerns, but we also offer animal-assisted therapy, music and art therapy, movement therapy, and other research-driven therapeutic approaches.

Position Summary:

The Discharge Planner is responsible for planning and coordinating activities related to discharge planning. Developing and implementing a plan of care designed to meet the patient’s post hospitalization needs while ensuring continuity of care. No supervisory function. This position also serves in the role of Court Liaison which will coordinate and complete all documentation with the Mental Illness Courts regarding OPC’s, MIW’s, APOWW’s, and court ordered medications.

Qualifications

Education: Master’s Degree from an accredited college or university, in Social Work or a clinical related mental health field preferred.

Experience: Minimum of three (3) years’ experience in a psychiatric health care facility with direct experience in discharge planning.

Additional Requirements: Crisis Prevention Trainings (CPI) within 90 days of employment and prior to assisting with a restraining procedure.
