Qualifications

• Associate degree in a relevant field with credentials to provide counseling services.
• Bachelor’s degree or higher preferred in some roles; licensure/certification may be required.
• CA counselor certification/licensure as applicable to the role.

Responsibilities

• Provides orientation to new patients to program rules, policies, procedures, and patient rights.
• Conducts Intakes, Discharge Planning, Initial Assessments, and Initial Treatment Plans professionally within guidelines.
• Involves patients in assessment and treatment planning to support recovery.
• Administers biopsychosocial assessments and evaluates various life areas.
• Develops individualized Treatment Plans using SNAP; determines clinical necessity of counseling services.
• Monitors changes, documents circumstances, and addresses relapse indicators.
• Complies with Medical QA and Clinical Risk Management policies; uses screening tools to assess status.
• Provides counseling and referrals, coordinating with external services and groups.
• Attends team meetings and completes required trainings; participates in community outreach as needed.
