Qualifications

• Associate degree in relevant field and required credentials to provide counseling services.
• Bachelor's degree preferred for some roles in Kern County.

Responsibilities

• Provides orientation to new patients to the program’s rules, policies, procedures, and rights.
• Conduct Intakes, Discharge Planning, Initial Assessments, and Initial Treatment Plans professionally and within time guidelines.
• Administer the biopsychosocial assessment and develop SNAPP-based treatment plans to meet patient goals.
• Monitor changes in patient circumstances and address relapse indicators to promote prevention.
• Provide professional counseling and referrals, and participate in team meetings and trainings.
