Job summary

The Meadow's is seeking a Staff Speech-Language Pathologist to evaluate and treat residents and to train facility staff. The role includes ongoing communication with families and the health team, documentation in medical records, and discharge planning.

Qualified candidates hold a state license in Speech-Language Pathology and preferably a Masters/Doctorate. Supervision of CFY or students is included in duties and professional development is encouraged.

Qualifications

• Bachelor's degree in Speech-Language Pathology; Masters/Doctorate preferred.
• Licensed as a Speech Therapist in the state; must maintain license in good standing.

Responsibilities

• Evaluate residents to determine speech therapy needs and rehabilitation potential.
• Develop and update individualized treatment plans to improve mobility, strength, balance and independence.
• Supervise CFY or students in resident care and related activities.
• Communicate with physicians, nursing staff, residents, and families regarding progress and discharge planning.
• Complete timely documentation in accordance with facility and payer requirements.
• Maintain safe treatment environment and educate residents and families on goals and home programs.
