Job summary

SCAN, a nonprofit health organization serving older adults, seeks an HEDIS Nurse for remote data abstraction and outreach. You will review records, coordinate with members and providers, and support quality initiatives per NCQA guidelines.

Qualifications

• 2–3 years of related healthcare experience.
• Knowledge of NCQA guidelines and quality initiatives.
• Ability to abstract data from medical records and document interactions.

Responsibilities

• Abstract clinical data from medical records per NCQA guidelines.
• Communicate with members and providers about Quality Initiatives.
• Enter findings into designated database.
• Review and pursue missing data to complete records.
• Coordinate with project manager and HEDIS staff.
