Job summary

Patient First is seeking an RN to serve as a telephonic care management resource for clinical teams in our Richmond area centers. You will build relationships with physicians, define quality measures, and coordinate care plans to close gaps in care.

Strong communication, triage, and care coordination experience are essential to succeed in this role. Responsibilities include patient education, collaboration with care teams, and maintaining complete documentation in the EMR.

Qualifications

• RN licensure required; RN, BSN preferred
• Ability to be licensed in all states Patient First operates
• Current CPR, ACLS, and PALS certifications; Telephonic triage experience expected
• Experience in Case Management or primary care coordination preferred
• Strong communication and interpersonal skills with teamwork emphasis

Responsibilities

• Serve as telephonic care management resource to clinical teams
• Define quality measures and care protocols with leadership
• Identify gaps in care from patient records and coordinate improvement actions
• Educate patients and care teams on preventative care and care coordination
• Coordinate change readiness and develop individualized care plans
• Monitor treatment plans and adjust as needed to improve outcomes
• Document patient self-management measures and progress in EMR
• Collaborate with physicians, nurses, and care team members to ensure continuity of care
• Assist with handling urgent calls within nursing scope of practice
• Promote patient engagement and activation in self-management
