Qualifications

• Unrestricted LMSW license in the State of Michigan.
• 4+ years of experience in community health care settings.
• 2+ years of case management experience.
• 1+ years of experience with MS Word, Excel, and Outlook.
• 1+ years of experience with Michigan Medicaid managed care.
• 1+ years of experience with medically complex pediatric members.

Responsibilities

• Assess, plan, and implement individualized care strategies.
• Manage the care plan as a single point of contact.
• Communicate required health information to stakeholders for coordinated care.
• Advocate for patients and families to support their choices.
• Provide telephonic care management to caseload, including special populations.
• Assist outreach to recently discharged members to prevent readmissions.
• Participate in interdisciplinary rounds and collaborate with Medical Directors.
• Support Quality and Population Health initiatives.
