Qualifications

• Unrestricted LMSW in Michigan is required.
• At least 4+ years in community health or clinical care.
• 2+ years in case management.
• 1+ year MS Word/Excel/Outlook experience.
• 1+ year with Michigan Medicaid managed care.
• 1+ year with medically complex pediatric members.

Responsibilities

• Assess, plan, and implement individualized care strategies.
• Manage care plans as single point of contact across the continuum.
• Coordinate with stakeholders to ensure timely quality care.
• Advocate for patient and family needs with the health team.
• Provide telephonic care management for caseloads.
• Support outreach after discharge to prevent readmissions.
• Participate in interdisciplinary rounds and QA initiatives.
• Collaborate with Medical Directors on complex cases.
• Support population health efforts and workflows.
