Qualifications

• Specialized Diploma graduate from an accredited nursing program is required.
• Knowledge of patient- and family-centered care and cultural respect.
• Active RN license in state of practice (Pennsylvania).
• Strong communication and collaboration with inter-professional teams.
• Proven ability to assess, educate, and discharge planning for patients.

Responsibilities

• Interacts with patients, families, and staff to deliver safe, compassionate care.
• Partners with patients and families to identify learning needs and priorities.
• Applies information technology to monitor care outcomes and support quality improvement.
• Collaborates within nursing and inter-professional teams for optimal care.
