Job summary

Responsibilities

Conduct comprehensive assessments to evaluate patients across physiological, psychological, developmental, sociocultural, spiritual and lifestyle factors.
Develop and implement individualized care plans, regularly evaluating and revising as needed.
Administer medications and treatments accurately per physician orders and protocols.
Educate patients and families about health conditions, treatments and self-care.
Collaborate with medical staff to coordinate and optimize patient care.
Maintain accurate documentation of patient information, care activities and outcomes.
