Qualifications

Responsibilities

• Modifies patient plan of care based on data collection and patient needs.
• Implements Evidence-Based Practice (EBP) in patient care.
• Delegates according to scope of practice.
• Verifies plan of care with the patient, caregivers, and interprofessional team.
• Provides education based on learning needs of patients and families.
• Promotes effective communication among interprofessional team members and patients.
