Sage Memorial Hospital in Ganado, AZ is seeking a Registered Nurse to coordinate care for patients and families, lead MTU staff, and provide direct patient care per nursing standards. The role emphasizes education, discharge planning, and continuity of care across settings.

The incumbent will perform assessments, develop care plans, document in EHR, and participate in code blue responses as needed. Arizona nursing licensure and BLS are required; ACLS/PALS within 45 days.

Qualifications

Associates of Science in Nursing required; BSN strongly preferred.
Two years of clinical experience preferred; new nurses may be considered with residency.
Valid unrestricted nursing license from AZ or a compact state.
BLS certification required; ACLS and PALS due within 45 days of hire.

Responsabilités

Assess needs and conditions of patients by gathering health information.
Develop and implement a care plan to achieve expected outcomes.
Provide safe, high-quality, patient-centered care while respecting privacy.
Maintain infection control standards and patient safety.
Lead and mentor MTU staff; collaborate with MTU Medical Director.
Document observations and nursing interventions in EHR.
Administer medications per orders and protocols.
Ensure accurate handoffs and communicate effectively with team.
Promote health promotion, disease prevention, and care coordination across settings.
Assist with pre-admission, admission, and discharge processes.
