Job summary

Concord Hospital Health System is seeking a Licensed Nursing Assistant (LNA) to provide direct and indirect patient care under the supervision of a registered nurse. The role includes assisting with activities of daily living, documenting patient status, and maintaining safe, clean care environments.

Required: LNA education certificate, NH LNA license, and BLS certification. High school diploma or GED preferred.

Qualifications

• High school diploma or GED preferred.
• LNA Education Certificate required with competency report.
• Current BLS for Healthcare Providers or equivalent.
• Current New Hampshire State LNA license.

Responsibilities

• Performs activities of daily living and personal care needs under the guidance and supervision of the Registered Nurse.
• Performs technical skills/procedures relating to physical, mental health and social needs of patients under the guidance and supervision of the RN.
• Documents care provided and reports to the RN observations of patient’s physical, social and mental status and basic patient data collected.
• Demonstrates adherence to safe patient handling policy.
• Demonstrates time management skills.
• Maintains the patient care areas to ensure cleanliness, safety and supplies of material and equipment.
• Receives delegated work adhering to the five rights of delegation.
• Performs job specific requirements and other duties as assigned.
• Serves as a preceptor for new employees.
