Responsibilities

• Performs comprehensive initial and episodic health assessments.
• Initiates and implements an individualized nursing and medical plan of care in collaboration with interdisciplinary team.
• Initiates diagnostic, therapeutic, medical and nursing actions with physician.
• Provides follow-up care including acute and routine examinations.
• Monitors and adjusts patient medications according to protocol or care plan.
• Orders laboratory and diagnostic procedures and documents results.
• Provides health education to patient and family.
• Triages patient problem calls requiring medical attention.
