RWJBarnabas Health is seeking a Full-Time Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to support the Primary Care APP Float Pool across North/Central New Jersey. The role includes on-site and telemedicine coverage with day shifts and regional onsite responsibilities.

Qualifications
- Master's degree or graduate level certification required.
- Current NJ APN license for APN track or PA license with NCCPA for PA track.
- Nurse Practitioner Certification or PA Certification is required depending on track.
- RN license is required for APRN/PA roles in some sites.
- BLS certification is required for all applicants.
- Cover letter or statement not specified in posting but required by some sites.
Responsibilities
- Perform comprehensive initial and episodic health assessments.
- Develop and implement individualized care plans with interdisciplinary team.
- Initiate diagnostic, therapeutic, and nursing actions with physician collaboration.
- Provide follow-up care, including acute and routine visits.
- Manage medications per protocol and provide patient education.
- Coordinate referrals and telemedicine/onsite coverage across sites.
