Job summary

RWJBarnabas Health, Primary Care Service Live, is seeking a full-time Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to support the southern New Jersey primary care network. The role entails regional on-site coverage with telemedicine support, day shifts, and a salary range of $125,000–$195,000 per year.

Qualified candidates must hold CDS and DEA, BLS, NJ APN, NP/PA certification and current NJ licenses.

Qualifications

• Master's degree or graduate level certification required.
• APNs and PAs with active licenses in NJ.
• NCCPA certification required for PA role.

Responsibilities

• Performs comprehensive initial and episodic health assessments.
• Develops individualized care plans with interdisciplinary team.

• Provides acute and preventive care and patient education.

RWJBarnabas Health, Primary Care Service Live is seeking a Full-Time Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to be part of the primary care service line providing coverage to southern NJ.

Shift:

Full Time Days

Monday-Friday

Regional Onsite Coverage and Telemedicine Coverage

Essential Functions:

• Performs comprehensive initial and episodic health assessments; in order to identify existing or potential health problems
• Initiates and implements an individualized nursing and medical plan of care in collaboration with members of interdisciplinary team
• Initiates appropriate diagnostic, therapeutic, medical and nursing actions in conjunction with physician
• Provides follow-up care including acute and routine examinations
• Assesses progress, identify problems, initiate preventive measures, recognize emergent problems, initiate proper care in collaboration with physician
• Monitors and adjusts patients medication according to protocol or medical plan of care
• Implements nutritional therapies, as appropriate
• Triages patient problem calls which requires medical attention
• Orders appropriate laboratory and diagnostic procedures
• Recognizes normal and abnormal laboratory findings and the pathology involved
• Documents information in patients charts following documentation standard
• Provides health education to the patient and family (e,g,, management of side effects, anticipatory teaching)
• Manages medical emergencies as indicated, and according to standing orders
• Provides monitoring and continuity of care between health care settings
• Provides patient and family counseling and reassurance in regard to psychological and or psychosocial issues
• Initiates proper referrals to private or community agencies
• Acts as a resource and teacher for other members of the health care team
• Shares knowledge and expertise with staff
• Collaborates with the nursing and medical staff at partner and affiliate sites to coordinate continuity of care
• Evaluates the quality of care as well as the quality of life for patients through assessment of patient response and recommend interventions, as indicated

Travel Requirement:

Employee will be expected to provide both on-site and telemedicine support and coverage for practices as needed

Provides coverage for all practice sites within the assigned regional territory

Mileage reimbursement will be provided for travel to assigned locations from primary department

Frequency of travel will be contingent on the practice need for the assigned location

Duration of coverage will be contingent on the practice need for the assigned location
