Job summary

Archimedes is seeking an RN Case Manager for Telehealth to improve members’ health via tele-pharmacy and to support the development of clinical outreach programs. The role includes coaching members remotely, guiding program offerings, and ensuring services align with client expectations.

Responsibilities include creating clinical pathways, updating content with guidelines, and coordinating with prescribers and staff to maintain a high-quality program.

Qualifications

• RN license in good standing with state board required
• 2+ years clinical experience including direct consultation
• Experience in a clinical call center is preferred
• Knowledge of healthcare industry practices and HIPAA regulations preferred

Responsibilities

• Develop clinical algorithms, pathways and call scripts for pharmacy staff
• Review/update algorithms and scripts yearly with updated guidelines
• Develop documentation standards for clinical outreach
• Collaborate with staff to maintain high quality, consistent product
• Provide clinical outreach to members, prescribers and providers to enhance care coordination
• Stay current with guidelines, literature, and medications and integrate into programs
