Korzen Health is partnering with a leader in tele whole-person care to bring integrated medical and behavioral-health support to patients with complex, chronic needs. We are seeking a part-time Nurse Practitioner to serve as the medical clinician within a collaborative, multidisciplinary care team.

This role is primarily remote, with LCSWs and care-management staff handling much monitoring, patient outreach, and coordination, allowing you to focus on advanced-practice medical care.

Qualifications

• Active New Mexico Nurse Practitioner license or eligible to practice in NM.
• Experience managing common chronic medical conditions.
• Comfort working with behavioral-health populations and care coordination.

Responsabilités

• Conduct initial telehealth medical evaluations and follow-up visits.
• Review medical history, medications, chronic conditions, and care plans.
• Prescribe medications and order labs or referrals as appropriate.
• Provide medical input and oversight for patient care plans.
• Coordinate with behavioral-health clinicians, primary-care providers, and specialists.
• Review significant findings from ongoing patient monitoring.
• Respond to issues escalated by the care team.
