Job summary

Korzen Health is seeking a part-time Nurse Practitioner to serve as the medical clinician within a collaborative, multidisciplinary care team, delivering telehealth services for patients with complex, chronic needs.

This role is primarily remote, with care-management staff handling monitoring, outreach, and coordination. You will focus on advanced-practice medical care and work alongside behavioral-health clinicians and other care team members.

Qualifications

• Active New Mexico Nurse Practitioner license.
• Comfort managing common chronic medical conditions.
• Experience with telehealth and/or care management.

Responsibilities

• Conduct initial telehealth medical evaluations and periodic follow-up visits.
• Review medical history, medications, chronic conditions, and care plans.
• Manage common chronic medical conditions within your scope of practice.
• Provide a limited amount of general primary-care support.
• Prescribe medications and order labs, tests, or referrals when clinically appropriate.
• Review clinically significant findings identified through ongoing patient monitoring.
• Respond to issues escalated by the care team.
• Provide medical input and oversight for patient care plans.
• Coordinate with behavioral-health clinicians, primary-care providers, specialists, and other care team members as needed.

Job description

Korzen Health is partnering with a leader in the tele whole-person care model to bring integrated medical and behavioral-health support to patients with complex, chronic needs. We are seeking a part-time Nurse Practitioner to serve as the medical clinician within a collaborative, multidisciplinary care team.

This is a primarily remote role built around a team-based model. It is not a traditional full primary-care panel position. Licensed clinical social workers (LCSWs) and other care-team staff handle much of the ongoing monitoring, patient outreach, between-visit support, documentation, and care coordination. This lets you focus on work that requires an advanced-practice medical clinician.

Key Responsibilities

• Conduct initial telehealth medical evaluations and periodic follow-up visits
• Review medical history, medications, chronic conditions, and care plans
• Manage common chronic medical conditions within your scope of practice
• Provide a limited amount of general primary-care support
• Prescribe medications and order labs, tests, or referrals when clinically appropriate
• Review clinically significant findings identified through ongoing patient monitoring
• Respond to issues escalated by the care team
• Provide medical input and oversight for patient care plans

Qualifications

• Ability to work alongside LCSWs and care-management staff rather than as a traditional standalone primary-care provider
