Korzen Health is seeking a part-time Nurse Practitioner to serve as the medical clinician within a collaborative, multidisciplinary care team. This primarily remote role supports a team-based model with LCSW and care-management staff handling ongoing monitoring and coordination.

Responsibilities include telehealth evaluations, medication management, and coordination with primary-care providers and specialists. Active NM NP license required; telehealth experience preferred.

Qualificações

• Active NM Nurse Practitioner license is required.
• Comfort managing common chronic medical conditions.
• Comfort working with behavioral-health populations.
• Ability to thrive in collaborative, multidisciplinary care with LCSWs.

Responsabilidades

• Conduct initial telehealth medical evaluations and follow-ups.
• Review medical history, medications, chronic conditions, and care plans.
• Manage common chronic conditions within scope of practice.
• Prescribe medications and order labs, tests, or referrals as appropriate.
• Coordinate with behavioral-health clinicians and other care team members.
• Provide medical input and oversight for patient care plans.
• Respond to issues escalated by the care team.
