Job summary

Pear Suite is a fully remote, mission-driven digital health company building a care navigation platform for community-based care across the United States. This Nurse Practitioner role starts PRN, offering flexibility to grow toward regular part-time or full-time as volume and services expand.

You’ll perform telehealth assessments, determine CHW referrals, and design individualized care plans that address clinical needs and social determinants of health.

Qualifications

• Active, unrestricted NP/ APRN license in state(s) of practice.
• Experience in primary care or telehealth.
• Spanish or other language fluency preferred.

Responsibilities

• Conduct telehealth clinical evaluations for acute and chronic health needs and social drivers of health.
• Determine clinical appropriateness for CHW services and refer for care coordination as needed.
• Develop individualized care plans addressing clinical needs and social determinants of health.
