Job summary

Duly Health and Care is seeking a Case Management Coordinator to provide on-site support for transitions to the next level of care. You will help schedule patients, perform outreach, track data, and maintain accurate documentation to support transition-of-care workflows.

The role requires knowledge of care coordination workflows, medical terminology, and proficiency with clinical systems, with an emphasis on teamwork, communication, and reliability.

Qualifications

• High school diploma or GED required.
• Courses in medical terminology or healthcare office management preferred.
• Minimum of one year of medical office or hospital experience required.

Responsibilities

• Provide on-site support to patients and hospitalists to facilitate transitions to the next level of care.
• Educate patients on discharge follow-up and ensure they receive necessary appointment information.
• Coordinate communication between hospitalists, patients, and service providers for smooth transitions.
• Maintain logs and documentation for scheduling, outreach, follow-up, and transition metrics.
• Escalate scheduling barriers and support hospitalist workflows.
