Job summary

ROWI Long Beach, California is seeking a dedicated Care Coordinator to manage the coordination of care for adolescents in our wellness centers. Under supervision of the Program Director, you will handle orientation, transportation, doctor visits, and scheduling, ensuring adherence to care standards and documentation in EMR.

The role emphasizes a boundary-driven, compassionate approach, with no evening or weekend shifts, and a benefits package including healthcare, PTO, sick leave, and 401k

Qualifications

• Minimum high school diploma; Bachelor's degree preferred.
• Two years of driving experience required.
• One year or more of experience working with kids in a therapeutic setting preferred.
• Experience with junior high or high school students is preferred.
• Excellent written and verbal communication skills.
• Ability to maintain boundaries with clients and staff.

Responsibilities

• Coordinate care for ROWI clients, including orientation, transportation, doctor visits, and scheduling.
• Ensure clients attend designated groups and participate in programming.
• Facilitate groups and activities as directed by the Facility Manager.
• Complete client orientation on admission day and conduct assessments as directed.
• Document shift reports and transportation/activity in the EMR within 24 hours.
• Maintain confidentiality and professional interactions with clients and staff.
• Drive clients and manage transportation scheduling as needed.
• Support facility cleanliness and daily lunch coordination.
