Beth Israel Lahey Health is seeking a Team Nurse RN to deliver high-quality direct patient care in outpatient primary care or family medicine. You will work in a team-based model, support Medical Assistant staff, and educate patients on preventive and chronic care.

This role emphasizes coordination with primary care providers to ensure patient-centered care. The position requires MA licensure to practice in MA, CPR certification, and 1–3 years of related experience.

Qualifications

• Graduate from an accredited Nursing Program required. Bachelor's degree preferred.
• License Registered Nurse in Massachusetts required and CPR certification required.
• 1–3 years related work experience; new grads considered.
• CPR Certification is required.
• Strong interpersonal, patient education and communication skills.
• Advanced skills with Microsoft Office applications (Outlook/Word/Excel/Access) and related software.

Responsibilities

• Coordinate and facilitate quality clinical management across the outpatient population.
• Collaborate with providers; support transitions between levels of care.
• Provide skilled telephonic triage, education, and care coordination.
• INR management and teaching for patients on Coumadin.
• Administer vaccines and medications per provider orders and policies.
• Document care in EHR; schedule tests and referrals as ordered.
• Monitor abnormal results and oversee inventory of supplies.
