Job summary

GT Independence, LLC is seeking a Team Leader for the Community Supported Living (CSL) program in Antigo, Wisconsin. The role provides hands‑on support to clients, mentors Direct Support Staff, and ensures adherence to each member's plan of care.

You will lead daily operations, coordinate with managers and case managers, document activities, participate in on‑call rotations, and uphold GT Independence values in all workplace practices.

Qualifications

• 2 years of experience relevant to the work performed.

Responsibilities

• Builds and maintains relationships with members served, funders, and community stakeholders.

POSITION SUMMARY

The CSL Team Leader position will provide quality service throughout the CSL program. The Team Leader will support the daily functions of the program by providing hands‑on support to clients and mentoring Direct Support Staff Members.

WORK ENVIRONMENT

• Work is performed in an office environment as well as community‑based settings, primarily in clients’ homes. May include light to moderate lifting, bending, and walking. Traveling is required.
