Job summary

GT Independence, LLC is seeking a Team Leader for Community Supported Living in Merrill, Wisconsin. This on-site role supports CSL clients, supervises Direct Support Staff, and coordinates with care teams to ensure adherence to plans of care.

Responsibilities

• Provides daily direction to Direct Support Staff.
• Follows written plan of care for each member in the program.
• Provides hands-on direct support to CSL program members.
• Collaborates with CSL Manager, Case Managers, and natural supports to complement the member's plan of care.
• Monitors workloads and re-assigns tasks as needed.
• Completes necessary documentation for each member served.
• Responsible for staff training with the CSL Manager.
• Builds relationships with members, funders, and community stakeholders.
• Participates in On-Call rotation responsibilities.
• Upholds GT Independence values in all workplace practices.
• Other duties as assigned.
