Job summary

Hospice of the Midwest is seeking a Hospice Team Coordinator in Woodbury, MN. This in-office role supports clinical leadership and care teams, maintains patient records, answers phones, and coordinates scheduling and equipment needs.

You will input data into electronic systems (HCHB) and assist with admissions, discharges, and HR tasks. Join a mission-driven team focused on compassionate care and community impact.

Qualifications

• At least two years of administrative healthcare work experience.
• Strong organizational and communication skills.
• Ability to maintain patient records and scheduling.

Responsibilities

• Provide administrative support to clinical leadership and care teams.
• Maintain organized patient records, office files, and supplies.
• Answer phones, manage mail, and assist visitors with professionalism.
• Track and order medical equipment and office supplies.
• Input data into electronic systems (e.g., HCHB) and set up clinician tablets.

• Support HR functions like maintaining employee records.
• Ensure compliance with hospice policies and Medicare regulations.

Qualifications
