Job summary

Southeast Orthopedic Specialists in Jacksonville, FL seeks a Team Care Coordinator to serve as the primary liaison between patients, providers, and the healthcare team to ensure smooth care delivery. • High school diploma/GED or equivalent (medical background or relevant coursework preferred).
• CMA certification or college degree in healthcare/anatomy is a plus.
• Strong organizational, typing, and computer skills.
• Excellent interpersonal and communication skills.
• Ability to multi-task, work under pressure, and maintain confidentiality.
• Typing speed of 40+ WPM; ability to work independently while supporting team efficiency.

Responsibilities

• Serve as the primary liaison between patients, providers, and the healthcare team to ensure smooth care delivery.
• Guide patients through their healthcare journey, helping them understand conditions, medications, and provider instructions.
• Support providers by handling administrative and clinical tasks that allow more face-to-face patient care.
• Document patient visits accurately, including history, procedures, exams, and provider plans.
• Assist with in-office procedures (injections, aspirations, suture removal).
• Complete disability/FMLA paperwork and call in prescriptions/refills.
• Schedule appointments and procedures; triage patient calls.
• Coordinate with WC adjusters, home health agencies, and nurse case managers.
• Manage provider schedules and submit/renew physical therapy orders.
