Position Summary

Reporting to the Team Manager, this position is responsible for entering clinical data into the patient information system to create and manage medical records, ensuring compliance with state, federal, and accreditation standards. The role supports the Team Manager, staff, and daily operations, providing clerical and non-team-specific functions in a timely manner.

Essential Functions
  • Enters and updates patient data, including status changes and demographics.
  • Communicates medication lists to MD or NP.
  • Performs medical record reviews and audits.
  • Coordinates with Admissions regarding new admission follow-ups.
  • Maintains recertification calendars and notifies nurses/MDs of outstanding items.
  • Prepares agendas, sets up meetings, and takes minutes for weekly IDT meetings.
  • Handles communication via phone, faxes, and emails with facilities, providers, patients, and families.
  • Manages documentation such as emergency lists, notices of elections, DME orders, and patient transfer information.
  • Attends meetings and committees as needed.
  • Performs additional clerical duties as required.

Experience and Education
  • Minimum two years’ experience as an administrative assistant.
  • Previous experience in home health or hospice preferred.
  • High school diploma required; some college preferred.
  • Knowledge of medical terminology and Microsoft Office.
  • Experience with clinical database data entry preferred.

Schedule
