St. John's United seeks a TCA Team Lead at the Transitional Rehab Center to mentor TCAs, coordinate training, audit documentation, and work with unit leadership to implement CMS-aligned policies and practices. You will promote teamwork, accountability, and excellence in resident care.

Responsibilities include staff mentorship, chart audits, scheduling coordination, policy development, and involvement in admissions and care planning, while maintaining high standards of service and patient safety.

Qualifications

• Prior CNA experience preferred.
• Montana CNA certification required.
• Current Med Aid Certification or ability to obtain within one year of hire.

Responsabilités

• Mentor TCAs during orientation and ongoing.
• Audit TCA charting for accuracy and CMS compliance.
• Coordinate TCA training, scheduling, and coverage.
• Develop and review policies with Unit Leadership for CMS alignment.
• Routinely round on residents to assess care quality.
• Assist with admissions and care plan meetings.
• Maintain inventory of supplies and coordinate paperwork.
• Uphold customer service standards with residents and families.
