Job summary

WestCoast Children's Clinic in Oakland, CA seeks an EMPOWR Case Manager to provide trauma-informed case management and advocacy for transition aged youth (16-24) in a hybrid, full-time role. The position requires collaboration with a multidisciplinary EMPOWR team to connect youth with housing, healthcare, education, and employment resources.

Responsibilities include community-based services, crisis support, and documentation in Medi-Cal workflows.

Qualifications

• Bachelor's degree preferred in a related field.
• At least 2 years of experience with system-involved youth.
• Experience in case management and rehab services.

Responsibilities

• Provide community-based trauma-informed support to transition aged youth (16-24).
• Deliver advocacy, case management, and rehabilitation services.
• Assist youth in accessing housing, health care, education, and employment.
• Develop safety plans and conduct risk assessments.
• Coordinate with families, teachers, social workers and others.
