Job summary

Pacific Clinics seeks a dedicated case manager to support clients under supervision, ensuring emotional and physical safety. You will assess needs, develop strength-based plans, and provide in-home and community services with a client-centered approach.

You will document services, coordinate with families and teams, and assist with housing stability and successful discharge. Bilingual abilities are valued; travel to client sites may be required.

Qualifications

• Bachelor's degree in Social Work or related field; two years experience in Public or Private sector Social Services, or two years experience with Transition Aged Youth, ages 16 to 24.
• Speaking, reading, and/or writing another language highly desirable.
• Life experience in money management, career guidance, housing, home management and transportation.

Responsibilities

• Provides initial and ongoing assessment of client strengths and needs; develops, reviews, updates and ensures implementation of strength-based service plans including safety plans, independent living skills and legal issues.
• Completes all documentation in compliance with agency and regulatory requirements.
• Meets with clients in home and community settings, developing a helping relationship and ensuring needed supports and services are provided.
• Provides training of independent living skills and parenting skills to clients (if applicable).
• Supports client to facilitate a successful placement and ensure compliance with agency and regulatory requirements.
• Works to achieve permanency; builds and supports family and community connections and natural supports, at request of client.
• Assists client to develop and acquire future housing prior to discharge.
• Maintains ongoing communication and coordinates with authorized representatives and other team members; participates in meetings.
